Holy Name Hospital - Teaneck, NJ
Fitness First, April 2004 - February, 2007

Developed 23,000 SF medically integrated, hospital affiliated health and
fitness center using a high level of technology to track and monitor member
outcomes and report back to physician\'s member progress. The center pre-
sold 1600 members during a 4 month pre-sale with contracted revenues over
$1.4 MM. While membership was open to the public, this facility also
concentrated on disease management programs to meet criteria set up under
the Federal Government\'s IRS 502 Tax Code.

Matrix Fitness Centers Management, LLC - Irvington, NY
Founder, August 2002 to December 2005
Matrix Fitness Centers Management partnered with St. Joseph Wayne Hospital
to develop a medically based, hospital affiliated health and fitness
centers. This 18,000 SF center operates in Pompton Lakes, NJ under the
name Matrix Fitness Centers.

Stamford Health System - Stamford, CT
Vice President, Ambulatory Services, July 2000 to May 2002
Responsible for outpatient business services for Stamford Health System and
directly for the clinical outcomes of the cardiac rehabilitation and health
and fitness business units. Business services include outpatient surgery,
wound care, family medicine, immediate care center, behavioral medicine,
diagnostic imaging, physician office space (tenants), corporate health,
employee health, integrative medicine, conference facilities, caf and
child care. All these services are housed in a 225,000 SF outpatient
services building that opened in May 2002.

TBG Development, L.L.C. - St. Louis, MO
Senior Vice President - Operations/Partner, 1997 to 2000
Matched feasibility study results with the operational and financial goals
of health and fitness center projects. Oversaw facility design, layout and
equipment selection to ensure their appropriateness for the target markets.
Supervised the company\'s sales and management teams, financial goals,
quality assurance and customer service programs. Oversaw company wide
operations that included over 450 employees, $40 million in revenues and in
excess of 400,000 SF of facilities.

Linda Evans Fitness Centers - San Ramon, CA
Executive Vice President - Operations, 1996-97
Responsibilities included developing a 5-year plan to open 200 locations in
the women\'s fitness business. Supervised 650 employees; managed a 20-
person corporate staff; directed sales for four regions; implemented
control systems for contract writing, personal training, information
management, purchasing, payroll and facility maintenance.

Signature Health (a subsidiary of Western Athletic Clubs) - San Francisco,
CA
President, 1995-96
Provided health and fitness services link between insurance
companies/corporations and the Western Athletic Clubs nine-club network.
Responsibilities included developing relationships with insurance carriers
to provide capitated programs to their subscribers. Directed corporate
sales for Signature Health products and club memberships to corporations in
the Houston, San Francisco and Seattle markets.

Cape Coral Hospital - Cape Coral, FL
Assistant Vice President, 1990-94
Responsible for the design and development of a $5 million, 42,000-square-
foot health and fitness center and operated two satellite facilities.
Served as President of a Hospital subsidiary named LifeTech and was
responsible for the development and management of health and fitness
centers in corporate, commercial and hospital markets.

The Sporting Clubs of America - Chicago, IL
National Fitness Director & Executive Director, 1988-90
Responsible for the development of all health/fitness products and medical
affiliations for a $200 million, nine-club organization. Supervised nine
directors and over 250 fitness instructors. Implemented a personal
training system generating $40,000 per month and a sales lead system
generating 270 leads per week. Executive Director responsibilities
included marketing, advertising, public relations, accounting and pre-sale
for a $35 million, 135,000-square-foot multi-purpose facility in downtown
Chicago.
