Brief Description:
The purpose of this Request for Response is to solicit providers of structural motor vehicle modification evaluations and consultations and driver evaluations for individuals who will require structural motor vehicle modifications. Qualified provider(s) will provide functional assessments and related written driver evaluations to include functional capacity profile and recommendations for structural vehicle modification needs of individuals with disabilities. Such functional assessments and driver evaluations are necessary for the Commission to determine the need for motor vehicle modification services, the nature and extent of modifications, the type of vehicle necessary, and the individuals ability to operate the vehicle safely. The Commission Motor Vehicle Modification Program is responsible for structural motor vehicle modification services to eligible individuals referred by Commission area offices including determinations specific to structural motor vehicle modification services.

Rules:
1) Calendar Rule Type: Submit a bid per the Solicitation requirements as long as the Close Date has not been exceeded.
 
2) Applicable Procurement Law: MGL c. 7, § 22; c. 30, § 51, § 52; 801 CMR 21.00Open and competitive procurement for Goods and Services. Evaluated based on criteria designed to determine the best value to the Commonwealth.
 
3)Responses Are NOT Accepted Online: Vendors wishing to respond must mail in their response in accordance with the solicitation documents
 
4)Only ONE Response Per Vendor is Allowed: Vendors can only submit one response for this solicitation
